Jack Cook, Jr. Jack Cook, Jr. 69, of Earlington KY, formerly of Evansville, passed away at Regional Medical Center in Madisonville on December 21, 2009. Jack was born April 29, 1940 to Jack Cook, Sr., and Lilian (Daywalt) Cook in Evansville. Throughout his lifetime Jack had several different occupations. He worked at Whirlpool for several years and owned several small businesses at different times over the years. Jack is survived by his loving wife of 12 years, Sue Ann, his sons Gary Cook (Susan), Billy Marshall, Anthony Marshall (Patty), his daughters, Glenda Perkins, Mary Patterson, Cathy Cook, 14 grandchildren, 3 great grandchildren, brothers, Bruce Cook, Dennis Cook, sisters, Mary Francis Voight, Edna Pauline Dewig, and several nieces and nephews. A memorial service will be 10:30 a.m. Monday, December 28, 2009 at Eastminster Presbyterian Church, 5501 Washington Avenue, Evansville. Visitation will be 9:30 a.m. to service time. Titzer Family Funeral Homes ~ Meyer Chapel, 100 E. Columbia St., Evansville (812) 424-9288 are handling arrangements.
